Based on the graphic in Figure 1.3 of the Wannamaker article, it looks like "subtractive dither" is a dithered quantized (digitized) signal mixed with the inverted non-quantized (*analog*) dither noise.

So in reality, the only way "subtractive dither" could be utilized is in playback where the conversion is to shorter wordlengths, with active dither applied both before truncation and after conversion. (The noise after conversion inverted.) It could *not* be utilized in standard CD or MP3 playback because the media itself would require additional *analog* tracks to store the inverted dither noise.
