I also don't like to mount TO220 devices flat on a circuit board unless they are sinking a small amount of current. I want them up in the air. The accessory heat sinks I use have two little bolts and bolt to the circuit board so the heat sink can be a convection chimney. Also most 2 and 3 watt metal film resistors that carry a hot current should be mounted up in the air especially if the board is ground planed on top.
