Your first diagram shows how AC mains supplies are organised. In fact, I believe Oz mains power is not quite organised the same way as yours bcoz we have 240v ... but it's the same general idea.

In your second diagram (from Rod Elliott's Web-site), you are making the assumption that the earth shown on the left - underneath the AC mains 'active' and 'neutral' - is the same earth as the one shown on the right ... which is the DC power supply '0v'.

In my amplifiers, the 'mains earth' (this is the earth tag on the IEC socket) is connected to a chassis bolt, for electrical safety reasons. This also means the steel case acts as a faraday shield and protects against RFI.

The DC power supply 'ground' - which the transformer centre tap wires are connected to - is a "star earth" which floats, relative to the chassis/mains earth.
