Tubes can go out suddenly: Once, after removing the tubes, I accidentally inserted two 6n7s (=6N7) instead of two 6sn7 - both tubes in SLCF stage. On power-up, one of them quietly blew out (grid burned through) - that was the middle, CF tube - but no damage to output stage.
