There's more than one way but try taking the output from the plate of the bottom tube of a mu follower. If high voltage swing is needed to drive an output stage (without sacrificing linearity) then substitute the plate R with a choke. The interesting thing here is that the choke needn't be high quality nor have a very high inductance as would be the case with a simple choke loaded grounded cathode stage.Rgds,
