Question on gain structure...

To achieve, let's say, +18dBu of headroom in your sound system. Should you run the full +22dBu of signal into your amp and adjust the sensitivity control to just clipping?
Now, say your speaker is rated at 200W, and the amp you've just tweaked for proper headroom is rated at 200W. When your running your system at nominal pro level (+4dBu)wouldn't your output be very low (not using all the power of the amp)? Or wouldn't I notice that difference and be protecting the speakers from any clipped signal at the same time...Assuming you need to use all of this power to get the proper level at the rear of the house...
