Stu,Based on what I read, your methods seem to be correct.. unfortunately, Ralph is out of town at the moment, and I can't compare notes with him.
One other thing to look at: are you keeping the line voltage stable as you bring the unit to full power? The M-60 operates in the class A2 region at full power, and thus current draw can increase markedly. Just a few volts of sag in the line can prevent the amp from reaching full rated output.
If you have a variac and second AC voltmeter available, it would be worth your while to monitor and correct line voltage as you approach full power.
