Current production Ei KT-90s have many quality problems. I don't recommend them, I won't sell them. Your tube ran away and took out the cathode resistor at least, you need to have it checked to ensure the bias voltage is correct to that socket, and see if there is any other damage.I've written over and over about KT-90 type III (or what some others call "type IV") being poor quality. Each time I do somebody writes back and says how there KT-90s are fine. They aren't fine more than likely, but they haven't failed catastrophically so it's assumed all is fine.
The reason it didn't fail for you in the tube tester is the same reason some people can run them - they are operated SO conservatively that they don't show the symptoms typical of the poor quality tubes.
