The input transformer wouldn't be very critical other than the split winding. You really wouldn't have to have a split secondary on the input tranny as you already have two signals 180 degrees out of phase with any single secondary. Newer transformer companies like Jensen don't wind anything much past 1:10 as a bandwidth issue. The output transformer is the hard one. I'd look at Lundahl or Sowter. Shielding is going to be paramount on the input tranny at least to -60dbv. Yes, I would definitely use DC at least on the first two tube filaments. I would rather use a different tube as the power output stage 12au7/5814a or maybe a 6FQ7/6CG7. Good luck! Ray
