Here's what I think may have happened.

The only way a head could be damaged in my opinion, is if the tape contained an extremely abrasive element, which would be highly unlikely.

What might have occurred is the tape could have been contaminated with a substance that rubbed off onto the heads. I know you said you cleaned them, but there still could be some schmutz on there, so you might want to try it again. I've played old tapes that left crud on the heads that took a couple of cleanings to fully remove. Use denatured 100 percent alcohol and scrub the heads, tape guides, capstans, pinch rollers, everything.
