The assumption here is that the splicing was done *in the receptacle outlet box. That's 'typical', that's 'probable'. It's not for certain.
OP didn't state age or description of the residence in question. Convenience receptacle splicing could easily have been done in overhead (attic/crawlspace) boxes w/ vertical drops, or underfloor (basement) splicing with vertical risers.
There's 1MM ways to skin a cat, there's 1.5MM ways to wire a dwelling, especially considering the quantity of electrified residences in the US since - as a rough start date: 1920. What's a certainty is that not every one of the residences in the US was wired by an experienced inside wireman fully knowledgeable in the Codes in place at the time. Let alone past or present occupants of a dwelling that think they are 'weekend warrior' self-proclaimed electricians that are going to 'add on to' or 'enhance' their residential wiring system.
I'm not surprised by anything anymore. I will 'never see it all.' I have an example in my neighborhood of Type NM Romex, run outdoors exposed, aerially w/o a messenger, on wooden posts maybe 8-10 feet above grade, and at a public facility.
