I have a 12ax7 labeled Bogen. Any idea who really made it? It flares up when you turn the power on.

The start up flash you've noted is quite common to older production "real" Mullard's for one. My NOS Mullard 12AT7's do the same. I believe there are other NOS or ANOS European small signal tubes that exhibit a flash on start up. Amperex maybe?
It could be Mullard. But not all tubes that flash at startup are Mullards. US-made early-'60s CBS 12AX7A also flash on startup. And not all true Mullards will flash. Bogen did utilize Mullard stock, however...
