Won't that cause problems? Because of the winding capacitance, I thought a transformer was designed for one particular end of the primary winding to be commented to the B+ supply (AC ground) and one particular end of the secondary to be connected to ground?

Crowhurst is thinking of push-pull, where the CT of the output transformer is connected to B+. So you can swap the plate connections.
For 2-stage SE amps, you are right. You must either add a stage or subtract one (i.e. "Schade" feedback...).
I'm clearly stuck in a single ended rut. :-)
