I cannot find the service slips, unfortunately. I don't know except that the caps were serviced and I remember he replaced the housing in the sockets.
Follow Ups:
I was just going down that check list. You know, leaking coupling caps, bias supply, bad connection at tube socket dropping the bias voltage, etc....
It has to be something, probably simple but over looked.
Do you remember is the output tubes in that channel were "red plating"?
