Never mind, I found it. The button on the back to switch from balanced to unbalanced was pushed in to the balanced position. DU-UH
Follow Ups:
We've all had those "DU-UH" moments. They sting the ego for a second, but in the long run they're far better than when you inadvertently destroy something while attempting to fix, or worse yet, 'improve' it.
I'd also like to thank you for listing your thorough trouble-shooting list in your initial post. I find it irksome when someone posts a brief, "My X isn't working". Seven people reply with suggestions, to which the original poster replies, "Yes, yes, I've already tried all that!". Umm, that might have been good to know in advance. And people wonder why others don't seem to be as helpful around here as they used to be.
Again, happy to hear that it was only a switch out of position, and that the repair of the power switch was successful.
