first thing I would do is to check if the counter weight setting is correct.

if the effect is seen on multiple records, it is definitely a mechanical problem. IF just on one record, sometimes blowing against the problem will cut a correct path out of a very slight defect

the rare exception might be a low compliance cartridge that is thrown out of the groove by sudden intense groove modulation, like the cannon fire in some recordings of the 1812 overture. Moving coil carts are often very stiff riding designs. the suspension compliance can often be looked up.
