The two larger diameter pins on a 45, 2A3, or 300B are connected to the filaments of the tube and as you suggest, orienting the 45 tube the same way as the 2A3 was installed with regard to the large diameter pins is correct. I would hesitate to take advise from a local technician without understanding that he has an understanding or experience with vacuum tube circuits, since most people in the trade today have had little if any experience with vacuum tubes, and even less with directly heated triodes like the 45 and 2A3.
