What your describing doesnt sound like its incorrectly wired..You mentioned about the pair of cathodes being tied together in the phase inveter but if its a long tail it will use the plate to drive one output tube and the cathode to drive the other so you need to find a schematic and I would assume its a filter cap from your description.
Here is what you can do if you dont have a scope..Put your dvm on AC volts and check each filter cap section to ground and if your any large AC voltage like 10 volts or higher,thats a good place to start.Typically a defective filter cap will produce hum regardless of volume setting.
