That KT-90 was acting up. 520 volts across the tube and 50 ma is a plate dissipation of 26 watts - that's just barely more than 1/2 the rated dissipation.Look through the plate holes (carefully!!) while the tube is running and see if you see the screen wires glowing white in a lot of places. If so, you found the cause of the problem.
