My guess would be that you need more regulation in the B+. A second choke stage, more capatance, or a constant current source to get the noise response you want.

Easy test to determine if you have a B+ Problem is to put in a switch in the B+ that allows you to cut the B+ but leave the heater supplies on. While the B+ caps discharge, you will have enough voltage to hear if the noise goes away or stays when you cut the B+. Then you will know the cause.
