If you are using high sensitivity speakers then you may need DC heaters otherwise AC should be fine . EF91 may require grid stoppers , but what is suggested in the book ? A 2k grid stopper should be ok for a 12SN7 cathode follower . More than likely if there is a 'buzz' rather than a hum , there will be a problem relating to layout . It could be that you have taken the 0V line from the 'wrong side' of the reservoir cap .
