It shouldn't have disappeared, it probably just didn't load for some weird reason. Unloading with the same name is a bug though that I've not bothered to fix as it's obscure and a pain and I'm lazy. In that case, it would overwrite the first image and you'd have 2 entries in your Gallery with the same image. Deleting the mistake would leave the good entry in the db, but delete the image which might explain what happened. Then you'd have to delete the one with no image and upload again.
