Could be a lot of things since you're running quite a ways outside of the S/PDIF spec for coax. The signal level is fairly low so you have to be careful with routing on long runs to make sure you don't get near any sources of interference, especially if using cables with questionable shielding. The coupler in the middle will also probably introduce more reflections due to the impedance mismatch and make the problem worse. You could possibly improve the situation with a change in termination on the receive end, but that's probably not an option on a nice DAC like that. Try it with just one of the cables and see if that works OK. And maybe try routing the cables differently. Or as a last resort, if you can, try a better cable of the complete length.
