The 12ax7 used in the 400 are some of the best ever made. NOS prices are downright scary and the originals last forever. Test them and only replace if you must. Buying the same brand used but tested isn't too painful.Based on what you said your problem might be in the volume control. Turning it off and on helps because it moves this control (next time just trying rotating it a few times without shutting down). If you find there is a particular volume settting where this tends to happen then it's even more likely.
